The Form M-1 Online Filing System is an electronic filing system for the Form M-1 annual report for multiple employer welfare arrangements. The system allows filers to complete the form and submit it at no cost. This system is an example of an E-Government initiative which uses improved technology to make it easier for citizens and businesses to interact with the government.
This search engine identifies Form 5500 and 5500-SF filings received by EFAST2. Filings for plan years prior to 2009 are not displayed through this website. This website provides the forms, schedules, and attachments submitted by the filer. Posting this information on the website does not constitute acceptance of the filing by the Department of Labor, the Pension Benefit Guaranty Corporation, or the Internal Revenue Service. You can view one plan filing at a time through this search engine based on the search criteria listed below. If you would like the raw data from all filings, including schedules, you can download Form 5500 datasets from EBSA's electronic FOIA webpage. The expanded Form 5500 search tool is also for filings with the Department since January 2010. In addition to searching for specific filings, user can search for filings using filters including: plan type, plan asset value, number of participants, employer plan types, business codes, form years, and locations. The number of search results generated increased to 5,000 filings. Users may export search results to a CSV file.
The Form M-1 Online Search tool allows users to find reports for Multiple Employer Welfare Arrangements (MEWAs) and Association Health Plans (AHPs) filed with the Department of Labor. The Form M-1 is filed annually by MEWAs, (including AHPs) and ECEs (Entities Claiming Exception). Copies of the Form M-1 filings are also available from EBSA's Public Disclosure Room at 200 Constitution Avenue, NW, Room N-1515, Washington, DC 20210, or by calling (202) 693-8673.
These two online search tools allow users to search and view Apprenticeship and Training Plan Notices or Top Hat Plan Statements for certain pension plans maintained by an employer for a select group or management or highly compensated employees filed with the Department of Labor. Results can be printed or downloaded to Excel. At 5.5K, the file size is reduced compared to 11K (5.5K = approx. 4MB/image, 11K = approx. 10MB/image). The image quality at 5.5K is almost the same as that of THETA V and THETA SC2, but with the newly designed wireless module the image data transfer speed to smartphones is about 1.5 times faster than THETA V, which is one of the biggest improvements in THETA X.
My implementation consists of three projects, described below, of which only the middle one is of any interest to most people. A character map reader This simply finds all of the files whose names begin with "EBCDIC-" in the current directory, reads them all in (warning of any oddities in the encoding, such as any non-zero byte having two distinct meanings) and writes a resource file out, ebcdic.dat. This is a console applicion built from a single C# source file. An encoding library This is a library built from two C# source files and the ebcdic.dat file generated by the reader. This library is all most users will need. More details are provided below. A test program This is a console application built from a single C# source file and requiring the library described above. Currently it just displays the encoded version of "hello" and then decodes it.
The following is a sample program to convert a file from EBCDIC-US to ASCII. It should be easy to see how to modify it to convert the other way, or to use a different encoding (eg from EBCDIC-UK, or to UTF-8).
A while ago, a reader contacted me regarding an extra translation table for EBCDIC-AT-DE, also known as EBCDIC-1141 or code page 00273. Unfortunately I don't have the time to rebuild a zip file with the .NET 1.1 binaries etc at the moment, but I wanted to make his efforts publicly available - if you download a new ebcdic.dat file and build using that, you should have the new code page available to you.
